Lettuce-Linked Outbreak Devastates Midwest
The primary infections stem from a massive nine-state outbreak affecting Illinois, Indiana, Kansas, Kentucky, Michigan, Ohio, Oklahoma, Pennsylvania, and West Virginia. Interviews with sickened individuals revealed a common thread: many had eaten at Taco Bell restaurants and consumed lettuce distributed by Taylor Farms de Mexico. The contaminated Taco Bell lettuce has since been recalled, but due to the parasite’s incubation period, new illnesses continue to emerge.
Michigan has borne the brunt of this Cyclospora outbreak. The Michigan Department of Health and Human Services reported a staggering 10,386 cases, resulting in 193 hospitalizations as of the latest update. Northwest Ohio is also reeling, with 2,926 cases reported, including 1,194 in Lucas County alone, according to the Toledo Lucas County Health Department. The sheer volume of cyclosporiasis cases in these regions has placed a significant strain on local healthcare systems and public health resources.
Second Cyclospora Outbreak Emerges Without Clear Source
As the lettuce investigation continues, health experts are monitoring a growing number of infections in North Carolina and other southern states. This second Cyclospora outbreak remains shrouded in mystery, with few details available about its origin or the specific food vehicle responsible. The lack of a clear link to a known source has complicated public health messaging and prevention efforts.
“[This] may well represent the second outbreak that’s not associated with the lettuce,” Craig Hedberg, PhD, professor of environmental health sciences at the University of Minnesota’s School of Public Health, told CIDRAP News. The FDA is actively working to trace the source of this new cluster, but the nature of the parasite makes the investigation particularly challenging. Unlike some foodborne illness pathogens, Cyclospora cannot be easily cultured in a laboratory, meaning investigators must rely heavily on epidemiological interviews.
Tracking Challenges and International Spread
Nationally, the CDC reports an additional 11,500 cyclosporiasis cases awaiting confirmation or further investigation to determine where patients contracted the parasite. This year, 423 hospitalizations have been linked to the gastrointestinal illness, with 45 states reporting cases. The widespread nature of the infections highlights the complexities of the modern food supply chain, where contaminated produce from suppliers like Taylor Farms de Mexico can be distributed across dozens of states in a matter of days.
Tracking Cyclospora is notoriously difficult. Much of the data investigators rely on comes from interviews about what patients have eaten in the weeks prior to falling ill. Furthermore, the illness has a delayed onset of one to two weeks after exposure, which creates a significant lag in reporting and complicates trace-back efforts. By the time an outbreak is identified, the contaminated food may have already been consumed or discarded.
The reach of the parasite extends beyond US borders. The UK Health Security Agency (UKHSA) announced an uptick in diagnoses, with 67 people who reported traveling testing positive. Of those, 48 had recently visited Mexico, staying at hotels near Riviera Maya and Cancun. In a typical year, the United Kingdom records only about 93 cases. The UKHSA anticipates a continued spike in cyclosporiasis cases linked to travel from Mexico and the United States, urging travelers to practice safe food and water consumption habits.
What Happens Next
Despite the overwhelming numbers, there may be a light at the end of the tunnel for the primary Cyclospora outbreak. Outbreaks of this nature often follow a predictable seasonal pattern, and cases may soon begin to decline as the contaminated products are removed from circulation and the growing season shifts.
“Typically, outbreaks of this type do have their natural history,” Hedberg explained. “We may well be on a downslope.”
However, the FDA’s trace-back investigation for the second, unidentified cluster remains a top priority. As health officials work to pinpoint the source of the southern states’ infections, consumers are advised to maintain strict food hygiene practices, including thoroughly washing fresh produce. The delayed onset of symptoms means that the full scope of both outbreaks may not be realized for several more weeks, and continued vigilance will be essential in preventing further spread of this persistent foodborne illness.
