The decision, announced September 8, 2026, allows clinicians to replace aromatase inhibitor therapy with camizestrant (marketed as Etcamah) the moment an ESR1 mutation is detected in circulating tumor DNA, provided the patient is already on a CDK4/6 inhibitor. The companion diagnostic, Guardant360 CDx, was approved alongside the drug.
Why ESR1 Mutations Matter
Roughly 30% of patients develop ESR1 mutations during first-line treatment with an aromatase inhibitor plus a CDK4/6 inhibitor. The mutation undermines the aromatase inhibitor’s effectiveness, leaving the cancer increasingly resistant to standard hormonal therapy. Until now, clinicians typically waited for radiographic progression before changing course.
Camizestrant works by a different mechanism — degrading the estrogen receptor rather than blocking estrogen production — so it remains effective even when ESR1 mutations have rendered aromatase inhibitors obsolete. The approval covers use alongside any of the three approved CDK4/6 inhibitors: abemaciclib, palbociclib, or ribociclib.
The SERENA-6 Evidence
The approval rests on SERENA-6, a randomized trial of 315 patients who had been stable on aromatase inhibitor plus CDK4/6 inhibitor therapy for at least six months when an ESR1 mutation was detected. Patients were split evenly between continuing the aromatase inhibitor or switching to camizestrant, with the CDK4/6 inhibitor unchanged in both arms.
Switching to camizestrant nearly doubled median progression-free survival — 16 months versus 9.2 months for those who stayed on the aromatase inhibitor. Patient-reported quality of life told a similar story: median time to deterioration was 21 months in the camizestrant arm compared with 6.4 months in the continuation arm. Overall survival data remain immature.
A Controversial Path to Approval
The approval was not unanimous. In April, the FDA’s Oncologic Drugs Advisory Committee voted 6-3 against it. Critics argued SERENA-6 never directly compared early switching — triggered by ctDNA — against waiting for imaging to show progression. Without that comparison, they said, the trial could not prove that acting on a blood test actually improves outcomes.
The FDA acknowledged the gap in its announcement, requiring confirmatory studies to “verify and describe clinical benefit.” Accelerated approval is contingent on those studies confirming the drug’s advantage in a real-world setting.
Safety Profile and Dosing
Camizestrant carries a boxed warning for QTc interval prolongation, particularly when combined with other drugs that extend the QTc interval. Additional warnings cover bradycardia and embryo-fetal toxicity, meaning prescribers must weigh cardiac history and reproductive status before initiating treatment.
The recommended dose is 75 mg orally once daily, taken with or without food, and continued until disease progression or unacceptable toxicity. The CDK4/6 inhibitor continues at the dose the patient was already receiving when the ESR1 mutation was first detected.
What Happens Next
The approval reframes how oncologists might monitor hormone receptor-positive, HER2-negative breast cancer. If confirmatory trials validate the ctDNA-driven approach, blood-based monitoring could become standard practice earlier in the treatment timeline, potentially sparing patients months on a failing regimen. Watch for the required confirmatory studies to report overall survival data, which will determine whether the accelerated approval converts to full approval. Clinicians will also need clear guidance on managing QTc risk in patients already taking multiple cardiac-affecting medications, and insurers will have to decide how readily they will cover Guardant360 CDx testing for patients who appear clinically stable.
