Baxter International and B. Braun Medical issued five recalls over a 10-day period in August 2026. The recalls affect products distributed to healthcare providers, hospitals, and distributors across at least 14 states. Neither manufacturer has received reports of injury or death tied to the recalled fluids, but the potential health consequences remain severe.
Scope of the Saline Contamination
Baxter International announced three recalls on August 25. The company identified fiberglass in its saline through microscopic analysis and filtration testing. Baxter distributed the affected saline to 14 states between July 31 and August 3, 2026. The company also recalled a lot of 70% dextrose injection on the same day after finding stainless steel particles floating in the solution.
B. Braun Medical initiated its first recall on August 19, pulling one lot of Excel Lactated Ringer’s injection. The company found particles identified primarily as cellulose cotton fiber, along with individual particles of cellulose wood fiber and polystyrene. A second B. Braun Medical recall followed on August 27 for saline distributed nationwide from February 27 to August 3, 2026. B. Braun initially identified the particles in this lot as zinc oxide before correcting the identification to iron oxide—a form of rust—on August 28.
A single lot of anticoagulant sodium citrate solution, used with equipment that collects plasma and platelets from blood donors, was also recalled for possibly containing fiberglass. This solution does not enter a vein directly.
What Happens if Particles Enter the Bloodstream?
The FDA’s draft guidance on inspecting injectable products outlines the dangers of introducing foreign particles into the bloodstream. Published case reports link these contaminants to infections, vein and artery blockages, microscopic blockages, internal organ abscesses, and inflammation at the injection site. Particles delivered into a vein generally cause more problems than ones injected under the skin or into a muscle.
“The concern is that foreign particles should never be introduced directly into the bloodstream,” said Robert Glatter, MD, an emergency physician at Northwell Health and assistant professor of Emergency Medicine at Zucker School of Medicine at Hofstra/Northwell. “Depending upon their size and number, particles could theoretically irritate a vein, trigger inflammation or an allergic response, or obstruct blood vessels. In a severe situation, obstruction of the pulmonary (lung) circulation could produce a pulmonary embolism or organ injury.”
Baxter’s recall notices specifically warn that fiberglass or stainless steel particles can cause blockages and clotting in blood vessels. These clots can cut off blood flow to vital organs, cause a life-threatening blood clot in the lungs, or lead to permanent organ damage or death. The particles may also irritate the vein, trigger swelling and redness, worsen an existing infection, and cause an allergic reaction.
Who is at Risk from the FDA Recall?
The FDA IV fluid recall directly impacts individuals currently receiving intravenous fluids at home or in nursing facilities. Baxter’s dextrose notice advised home care facilities that used the recalled lot to compound IV nutrition to contact their patients, instruct them to stop using the solutions, and replace them with fluids from a different lot. People recently hospitalized may also be affected.
Certain patient populations face a higher risk of harm from particulate contamination. The FDA’s December 2021 draft guidance lists conditions that elevate this risk, including a personal or family history of blood clots, recent major surgery, cancer, trauma, active infection, autoimmune disease, and diabetes-related vascular disease. Children and older adults are also at an increased risk.
“The recent recalls concern the manufacturing quality of particular lots — not the basic safety of saline itself,” Glatter noted. “Normal saline remains a fundamental component of hospital care. These recalls instead highlight why sterile manufacturing, visual inspection, lot tracking and rapid removal of potentially contaminated IV products are essential patient-safety safeguards.”
What Happens Next
Hospitals and healthcare facilities must identify the affected lot numbers, remove them from inventory, and halt their use immediately. Patients concerned about the FDA recall should contact the hospital, infusion center, or healthcare facility where they received the IV. The facility can determine whether the saline came from one of the recalled lots.
Patients should monitor the IV site for increasing redness, warmth, swelling, significant pain, drainage, or red streaking, especially if accompanied by fever or chills. These symptoms can indicate vein inflammation, fluid leakage into surrounding tissue, or infection.
Glatter advised that more serious symptoms warrant immediate medical attention. These include new or unexplained shortness of breath, chest pain, fainting, coughing up blood, severe weakness, or sudden neurologic symptoms. Patients with significant heart or kidney disease should also watch for worsening shortness of breath or marked swelling, as excessive fluid administration can contribute to fluid overload.
